Good Faith Estimate (No Surprises Act)
You have the right to receive a Good Faith Estimate explaining the expected cost of your therapy services. I provide a clear estimate before we begin and am always happy to answer questions about fees. This helps ensure there are no financial surprises and that you feel confident and informed from the start.
Insurance Policy
Follis Wellness is an out-of-network practice. I do not bill insurance directly, but I can provide a monthly statement (a “superbill”) for you to submit to your insurance company for possible reimbursement.
Working outside of insurance allows us to protect your privacy, choose the most effective treatment approach together, and work at a pace that supports your growth — not the limitations of a diagnosis or insurance schedule.
Telehealth Policy
Telehealth sessions are available to individuals located in Missouri at the time of each appointment. Sessions are conducted through a secure, HIPAA-compliant platform. To participate in telehealth, you’ll need a private space and a stable internet connection.
Privacy & Confidentiality
Your privacy is important. All sessions and communications are confidential and protected under HIPAA and state law. I only release information with your written consent, except in rare situations where disclosure is required to protect your safety or comply with legal requirements. We will review these circumstances together during the intake process.
Communication Guidelines
Email and voicemail are used primarily for scheduling or brief administrative questions. I do my best to respond within 1–2 business days.
I am not able to provide crisis support by phone, email, or text. If you need immediate help, please call 988, contact 911, or go to the nearest emergency room.Crisis & Emergency Support
Follis Wellness is not an emergency service. If you are in crisis or experiencing thoughts of harming yourself or someone else, please use one of these resources:
988 Suicide & Crisis Lifeline
911 Emergency Services
Your nearest emergency department
These services can provide immediate help when you need it most.
Cancellation & Missed Sessions
Your session time is reserved just for you. Cancellations made with less than 24 hours’ notice — or missed appointments — will be charged the full session fee. I understand that emergencies happen and appreciate as much notice as possible.
Fees & Payment
Payment is due at the time of service. I accept major credit cards as well as HSA/FSA cards. You will always know the cost of sessions up front, and there are no hidden fees. A Good Faith Estimate is provided upon request or at the start of services.
Scope of Practice
I work with adults seeking outpatient therapy for anxiety disorders, OCD, and related challenges. I may recommend a higher level of care or refer out if:
There is active suicidality or recent attempt
There is active substance dependence or withdrawal
There is untreated psychosis or mania
Intensive or residential treatment is needed
This helps ensure you receive the level of support that best fits your needs.
Professional Boundaries
To maintain a safe and effective therapeutic relationship, I keep clear professional boundaries. This means I do not engage in dual relationships outside the therapy space and do not provide mandated or forensic evaluations.
Treatment Expectations
Therapy is a collaborative process. We’ll work together to set goals, choose evidence-based approaches that fit your needs, and check in regularly about what’s helping. Your voice matters — and your feedback helps shape our work together.
Treatment Outcomes
Every person’s healing journey is unique. While evidence-based therapies are highly effective, outcomes vary depending on many factors, including readiness, consistency, and life circumstances. My commitment is to provide culturally responsive, evidence-based care tailored to you, and to track progress together in a way that feels supportive and transparent.
